The VA Central Iowa Health Care System provides a full continuum of care between the Des Moines campus and our Community Based Outpatient Clinics (CBOC's) and is one of the Medical Centers that make up the VA Midwest Health Care Network (VISN 23). Employee selected may be expected to perform duties at other duty sites within VA Central Iowa Health Care System based on the needs of the organization.

Licensed Practical Nurse - Restorative Palliative Hospice

The Licensed Practical Nurse assigned to the Community Living Center, is responsible for providing care to a Veteran population with widely diverse conditions/diagnoses. He/she is responsible for direct patient care activities under the supervision of the Registered Nurse. Common duties of the position include, but are not limited to:

  • Perform duties of a Licensed Practical Nurse at a competent level.

  • Implements orders and provides care to a variable resident population.

  • Performs duties/assignments with supplemental verbal instructions that are given by a Registered Nurse.

  • Observes, identifies, and responds to resident's need for care.

  • Observes for physical and/or emotional changes in resident's condition promptly and accurately notating deviations from normal to the RN and/or Physician.

  • Responsible for supporting factors that impact customer satisfaction.

  • Considers all characteristics of the individual including age, life stage, state of health, race and culture, values, and previous experiences to provide optimum veteran centered care.

  • Administers medications and procedures per established policies and guidelines.

  • Collaborates with interdisciplinary team members to positively influence veteran care outcomes.

Work Schedule:Evenings, 8hr shifts (3:30pm-midnight) or Nights, 8hr shifts (11:30pm-8:00am) All tours will rotate weekends and holidays.

Basic Requirements:

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. **

  • Education: Must be a Graduate of a school of practical or vocational nursing approved by the appropriate State accrediting agency and/or the NLN at the time program was completed or completion of education or training in the military service which is accepted by the Licensing body in the jurisdiction in which the individual is licensed qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements

  • Licensure: Possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e. Puerto Rico) of the United States, or District of Columbia

  • English Language Proficiency: Must be proficient in spoken and written English (LVN/LPNs appointed to direct care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)

Grade Determinations:

  • GS-4 (Experience): Must have completed at least (6) six months of qualifying experience as an LPN or LVN.


  • GS-4 (Education): Must have graduated from an approved school and have (1) one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures.

OR (Education Continued) Must have graduated from an approved school of at least 24 months duration.

  • GS-5 Must have completed at least (1) one year of qualifying experience at the GS-4 level.

  • GS-6 Must have completed at least (1) one year of additional qualifying experience.

Preferred Education, Experience, and Skills:

  • Flexibility in schedule and assignments for a 24/7 facility

  • Highly motivated, self-directed individual

  • 1-2 years of experience

  • Poised and articulate in communicating with individuals of varied education and ethnic backgrounds

  • Strong background in medical documentation

  • Collaborative and effective team member

  • Good organizational and time-management skills

  • Previous Hospice/Palliative Care Experience

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, kneeling, pulling, and pushing. Transferring patients and objects may be required. Must be able to lift at least 45 pounds. The candidate may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The candidate may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The candidate will be working closely with others and may have protracted or irregular hours of work.
