The Resources for Human Development Assertive Community Treatment (ACT) team provides recovery-focused, multidisciplinary, intensive support for people in recovery from mental illness. RHD ACT teams bring together counselors, psychiatrists, registered nurses, case managers, housing, and vocational specialists to help individuals with serious, persistent mental health challenges maintain employment and social relationships, live independently and manage their health.

The Assertive Community Treatment (ACT) Registered Nurse provides the medical and psychiatric assessment and direct care as appropriate to state laws and as prescribed by a licensed physician. These services are designed for the consumers diagnosed with a serious and persistent mental illness and having met the criteria for participation in the ACT program.
These responsibilities of the nurse on the ACT team include: initial nursing assessment of the medical and psychiatric status of the consumers, collaboration with the physician, instruction regarding disease process, medication, symptom management and activities and interventions to enhance well-being. In addition, the Registered Nurse will assume an active role in the team process designed to maximize collaborative efforts.
Essential Duties and Functions
Support and promote the mission statement and values of Resources for Human Development Assertive Community Treatment Team.
Obtain the consumer medical history and assess for level of care.
In collaboration with the consumer and physician formulate a service plan designed to promote good health practices and participation in the recovery process.
Conducts the review of the service plan with particular focus on psychiatric and physical health stabilization/management.
Assists in obtaining and/or interpreting medical information to improve the team member s knowledge base regarding consumers health status, needs, and strengths.
Coordinate Clozaril reporting to adhere to regulations and requirements.
Perform skilled nursing activities as prescribed by the consumer s physician and within the purview of the law.
Interprets and carries out physician orders accurately and promptly.
Initiates referrals to other disciplines and other agencies.
Consults with team leaders and Clinical Coordinator regarding high risk, problem prone consumers.
Participates in specialty trainings for team members and consumers.
Work in a collaborative and collegial manner with other staff both inter- and intra- agency in assisting the consumer and the program to meet identified goals.
Participate in on-going planning and the development and implementation of a Continuous Quality Improvement system for the ACT program.
Perform other duties as assigned within the general functions of the position.
RN/BSN degree from an accredited nursing school with a current license to practice in the state of Iowa.
A minimum of one year experience in a health care setting provding care to individuls with mental illness.
Valid Iowa Driver s License
Understanding of and interested in working with persons with severe mental illness in the community and home environment.
The team registered nurse must be knowledgeable, flexible, and able to respond appropriately and effectively in a crisis situation.
The position requires the individual to be assertive and able to communicate well with staff, consumers being served and the various persons and organizations with which the team has contact in the community.
The team nurse must be organized, goal oriented and able to follow through with numerous and varied responsibilities.
