A Typical Day for an RN Lead with Heritage House:

• This position will provide a lead support to the Assisted Living Clinical Department within Heritage House.

• Lead RN will manage all medication planner set up and will ensure medications are listed in an easy-to-understand format in PCC for staff administration.

• Lead RN will audit new and current resident assessments completed by nursing, assuring that in accordance with Wesley policy a preadmission, admission, and required follow up assessments such as change of condition assessments are completed within required time frames and will notify Director of any discrepancies.

• Lead RN will audit PCC/POC compliance and will follow up to assure orders, medication changes, labs, follow ups, and appointments are put in PCC, noted and carried out in a safe manner.

• Lead RN will follow up on any changes in resident level of care requests, Hospice waiver requests and change of condition requests from Director to assure AL nurses are completing as indicated.

• Lead RN will follow up on medication errors and with assistance from Director, implement changes to avoid potential errors in the future.

• Lead RN will assist the Director with follow-up on any incident reports to assure they are entered in a timely manner, that a head-to-toe assessment and progress note was completed, and documentation completed in PCC as directed by policy.

• Lead RN will audit CDAC compliance of RA’s and get reports submitted to office manager in a timely manner.

• Lead RN will delegate out daily tasks to RN as directed by Director and will follow up on all RN duties daily to assure that standards of care through Welsey Life are being met.
