Green Hills AEA Service Coordinator in Avoca, Iowa

Green Hills Area Education Agency is looking to fill a Service Coordinator position. This area will cover the following districts: AHSTW, ATLANTIC, GRISWOLD, RIVERSIDE, HARLAN, IKM-Manning.

Qualifications:High School diploma or equivalent. Parent of a child with special needs or knowledge of early childhood development and family dynamics.Ability to communicate well both orally and in writing. Excellent organizational skills. Drivers license and personal vehicle required.Ability to make connections with students, families, schools, and agencies.

General Summary:To partner with families to meet their unique needs within an integrated system of early intervention services and within natural communities of support.

Essential Functions and Responsibilities:

Coordinates EA and other community services by serving as a single point of contact in helping parents obtain early intervention services, releases of informations and other services/resources (medicaid and waiver assistance, medical services, diagnosis)

Assist parents of eligible children in gaining access to early intervention services and other services identified in the IFSP (Individualized Family Service Plan).

Continuously seeking the appropriate service(s) and situations necessary to benefit the development of each child being served for the duration of the childs eligibility by identifying the family and childs needs, concerns, formal and informal networks

Facilitate and participate in the development, review, and evaluation of IFSP, as well as, coordinate and monitor the delivery of services.

Inform families of the availability of advocacy services.

Facilitate the development of a transition plan to preschool services, if appropriate.

Effectively participates in team meetings, staff development and professional growth activities (demonstrates positive interpersonal skills, decision-making skills)Follow the Agency-prescribed intake process.

Complete and submit all IFSP paperwork in a timely manner. Seek ICD-10 diagnostic codes for medicaid billing, bill medicaid monthly, send Thank You notes to referral sources, and upload relevant documentation into the IFSP system.

Demonstrate effective planning and organizational skills to meet job responsibilities

Perform other duties as assigned.

Physical Requirements:

Regular attendance is required.

Ability to travel within assigned schools

Ability to work flexible hours, including minimal evenings and weekends

Requires good eye/hand/finger coordination to keyboard and to operate office machines. Ability to communicate effectively orally and in writing. Good concentration and visual skills required. Normal, routine levels of activity related to bending, carrying, hearing, lifting, reaching, sitting, standing, vision and walking and may also involve above-average levels of activity at times that cant always be anticipated.
