Position Title: RN Team Lead, Care Coordination
Department: Care Coordination
Purpose: Oversees daily operations and staff assignments for Care Coordination activities. Provides clinical direction and feedback to staff regarding performance of day to day care activities, and support to staff in advanced problem solving of clinical cases. Assists manager in a variety of tasks, including: staffing and scheduling, budget development, productivity and performance management, new staff orientation, compliance of staff to department policies, procedures or workflow expectations/practice guidelines, performance improvement activities, data collection/analysis, preparation for compliance with regulatory agencies, identifying program development needs and assisting in implementation. Assumes responsibility for day to day activities when Director absence. Serves as role model with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated as patients transition from GMC site of care. May need to be assigned or to cover a subset of patients and address care coordination needs (E.g., utilization review, discharge planning, progression of care).

Supervisory Responsibility: Advanced Direction: The job requires some accountability for scheduling, assigning, or coordinating work. The employee will check the quality of work, provide guidance, instruction, training, and direction to others. The job requires some formal or official supervisory responsibility. The individual serves as a group leader and acts in an assistant supervisory or lead role. In addition, the employee is expected to provide information or suggestions on human resources matters.
Materials Responsibility: Moderate: Work requires moderate responsibility for material resources. Examples of resources could include operating budgets for a work unit or department, specialized equipment, costly or unusual materials or supplies, large amounts of cash or other material assets. The employee has a moderate amount of control over these resources. The cost of errors might result in moderate damage, waste or financial loss. The difficulty, variety and depth of problems associated with these material resources is moderately complex.

Education: Bachelor's Degree
Field Of Study: Nursing
Training Preferred: Certification in Case Management, Professional Utilization Review or Managed Care; DRG/CPT knowledge; Interqual Admission/Continued Stay criteria knowledge
Licensure/Registration: Registered Nurse in Iowa or Illinois required upon hire. Second state license required in 6 months.
Experience: More than 3 years experience required.
Interpersonal Skills: Interaction is with a variety of people inside or outside the organization. Communications are of moderate difficulty and sensitivity. Contact with others may involve detailed & lengthy dialogues & exchanges of information. Requires a moderate amount of interpersonal skills. Interactions involve dealing with moderately complicated problem situations or stressful encounters.
