For over 50 years, Eyerly Ball has provided a full continuum of mental health services and treatment to meet the needs of our community, from family and individual counseling to outpatient care for all ages. Our innovative solutions provide comprehensive, effective mental health services, including: outpatient, residential, community outreach, care coordination and crisis services.
At Eyerly Ball, we are currently looking for a Case Manager on the ACT Teamin the Des Moines office. The ACT Case Manager will work directly with clients of the Assertive Community Treatment Team (ACT) by using the evidence based practice model of ACT designed by NAMI to provide comprehensive and wrap around services to address client goals of mental health, physical health, housing, employment, social goals, and other goals as identified by the client. In this role, qualified professionals will work successfully with the client and fellow team members to meet outcome objectives and standards put forth by Polk County Health Services (PCHS).

Bachelor’s degree in social work, psychology, counseling or related field from an accredited college or university.
Prior work related experience with population served, preferably in a psychiatric team environment.
Must maintain a minimum of $100,000 per person and $300,000 per accident auto insurance liability coverage and must be able to present proof of that coverage.
