Director Community Transform Program
Location: Remote - Northeast Region preferred
We have two (2) positions available - see below descriptions for each.
The Director Community Transform Program is responsible for strategy, oversight, execution and ongoing management of the Care Delivery Transformation (CDT) programs and priorities.
The Director Community Transformation Program (Success Management Team) is responsible for a team which is comprised of the Care Delivery Transformation team associates responsible for data and analytics, orientation and continuing education for both the CDT team and provider partners, and core services which support the CDT Care Consultants. Primary duties may include, but are not limited to:
Leads the development and implementation of learning collaboratives, webinars, and toolkit enhancement, including national and local strategy.
Leads and supports the development of communication documents and training criteria related to program success measures.
Leads and supports the development of documentation which supports internal and external presentations as well as tracking and monitoring the value and success of program implementation.
Collaborates with Care Delivery Transformation Directors and Program Directors related to all program development and internal/external communication.
Hires, trains, coaches, counsels and evaluates performance of direct reports.
Requires a BA/BS
5 years of experience in external client facing program management, primary and specialty care transformation, quality improvement, and experience in a primary/ambulatory care setting; or any combination of education and experience, which would provide an equivalent background.
5 years experience leading multidisciplinary teams.
Experience in a payer, provider or health careconsultant position which focused on value based care models and providerpartnerships required.
Broad range of experience with Commercial, Medicare and Medicaid programs preferred.
Experience in strategic planning, programdevelopment, and the ability to lead groups is required.
Understanding of the data and analytics whichsupport value based care programs is required.
Experience leading complex projects involvingmultiple matrixed workgroups across a large organization preferred.
MBA preferred in relevant field of study.
Project management experience preferred.
Minimal travel required - approximately 10%.
