Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. General grooming, including skin care, maintain skin integrity/dressing application and changes as appropriate 6. Bed-making (unoccupied, occupied, surgical) Positioning and Movement of Patients 1. Maintain body alignment of patients 2. Position and turn patients (proper use of assistive devices) 3. Walking patients (proper use of assistive devices) 4. Range of motion (ROM) with patients 5. Transfer techniques (lifting, moving, transferring - use of all devices) 6. Caring for patients in restraints and protective devices/appropriate use of 7. Assists providers and nurses in positioning patients for bedside procedures Nutrition and Fluids 1. Preparing patients for meals 2. Assisting and feeding patients, noting training for special precautions 3. Checking and recording intake and output (I&O), including calorie counts 4. Reporting to RN if tube feeding or IV fluids are running low 5. Providing water, ice and snacks to patients Elimination 1. Observing, reporting and recording patient bowel and bladder function 2. Washes hands appropriately before and after patient care 2. Follows Standard Precautions 3. Uses personal protective equipment as needed (gowns, gloves, goggles, masks, etc.) 4. Follows isolation procedures 5. Recognizes emergency situations, calls for help, initiates emergency procedures until help arrives and continues to be available to the response team until the emergency is resolved 6. Adheres to safety measures related to patients at risk for falling 7. Adheres to all facility safety regulations 8. Maintains a safe, clean, environment (patient room, storage areas, utility rooms, hallways) 9. Operates all equipment safely 10. Suicidal and Homicidal Ideation precautions implemented ( sweeping room and unit area of any potential items of danger); assists in initiation of and monitoring of patient with seclusion/behavioral management as directed, appropriate use and care of protective/restraint devices 11. Maintains privacy and dignity of patients 4. Safe guards confidential patient information Procedures 1. Adjusts/applies/monitors appropriate oxygen delivery device 2. Applies oxygen head on new O2 tank or to wall in the patient room 3. Observes, reports and records drainage to the LPN/RN 4. Performs, records and reports blood glucose testing using the finger stick method at the bedside 5. Sets up/monitors/records suction canister /equipment/supplies in the patient room 6. Collects and delivers specimens, as directed 7. Applies cardiac monitor leads to the patient's chest/performs EKG as applicable by unit and competency 8. Turns on the bedside cardiac monitor Documenting and Reporting Activities: 1. Participates in nursing care planning by providing information received by the patient/significant other to the appropriate team members. 2. Record observations and treatments on the patients' record according to approved format 3. Reports changes in patients' condition to the appropriate team member. 4. Motivation/Patient Teaching Activities: Through emphasis on good health habits, the employee provides the needed impetus for participation in activities of daily living with the ultimate goal of achieving self-care status at the highest level of patient's capability. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education or Training: One year above high school with courses related to the Nursing Assistant occupation. Completion of an intensive, specialized, occupation-related course of study of less than one year as a Nursing Assistant may also meet in full the experience requirements for GS-3. English Language Proficiency: Nursing Assistants must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 3, section A, paragraph 3j. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Preferred Experience: Mental health experience desired. Grade Determinations: GS-3 Experience: None beyond the basic requirements. GS-4 Experience: One year of specialized experience as a NA or experience in another medical field (licensed practical nurse, health technician, hospital corpsman, etc.), performing technical and nursing skills requiring knowledge of the human physical and emotional states, therapeutic communication, and technical skills required for basic and intermediate treatments; OR, Education: Successful completion of 2 years above high school in an accredited institution with courses related to nursing care, health care or in a field of study appropriate to the specialization of the position, such as education in a program for psychology, psychiatric, or operating room technicians; AND Demonstrated Knowledge, Skills, and Abilities (KSAs). In addition, the candidate must demonstrate the following KSAs: 1. Ability to provide a range of patient/resident care and contribute to the completion of the nurse care plan and recovery of patients/residents. 2. Ability to observe patients/residents' or resident's physical or emotional status and refer deviations and/or problems to supervisor in a timely manner. 3. Ability to properly use equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, blood glucose monitoring machine, specimen collection, etc. 4. Ability to communicate effectively with patients/residents, their families and other health professionals. GS-5 Experience: One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment. In addition, the candidate must demonstrate the professional KSAs in subparagraph (c) below; OR, Education: Successful completion of a 4-year course of study above high school leading to a bachelor's degree that included 24 semester hours of courses related to health care or possession of a bachelor's degree; AND Demonstrated KSAs 1. Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse. 2. Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new NAs by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting. 3. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-03 to GS-05. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infectious patients and contaminated materials and may be required to don personal protective clothing in isolation situations. The incumbent will be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations; able to shift priorities based on patient needs.
