Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.
Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.
As a Referral Coordinator you will be responsible for facilitating and tracking assigned patient referrals to outside service providers. Serves as the liaison between Primary Health Care, Inc. and community service providers. Coordinates with PHC team members and providers to ensure a positive experience for all patients. Demonstrates iCare values in daily interactions.

What You Will Do
Coordinates assigned referrals made from Primary Health Care, Inc. locations to outside service providers. Ensures that high priority referrals are processed in a timely manner in accordance with workflows. Ensures appointments are scheduled and kept by communicating with patients and service providers.
Identifies and problem-solves any barriers that would prohibit or delay patients from receiving necessary care. Tracks details and reasons for unkept appointments. Participates in continuous quality improvement initiatives.
Collaborates with Patient Care Teams to ensure efficient referral processes. Serves as a liaison amongst patients, providers, nurses, and partnering agencies.
Documents work in accordance with established workflows.
Qualifications You Need to Bring
Required:
High School Diploma or GED.
A minimum of 1-year relevant work experience in medical field or six months experience working at PHC.
Demonstrated knowledge of medical terminology.
Organization and prioritization and skills with ability to manager multiple priorities in a high-volume environment.
Excellent verbal and written communication skills.
Results and detail oriented with ability to take initiative and work independently.
Collaborative and team-oriented working effectively with diverse groups.
Demonstrated proficiency using Microsoft Office applications, including Word, Excel, Access, internet and ability to work with database programs such as Electronic Medical Records (EMR).
Preferred:
Medical terminology course work.
Experience in referral management.
Work experience in a community health center.
CMA or LPN.
Able to communicate verbally and in writing in English and Spanish.

Your experience and skills determine your base pay. The hiring range for this position is typically $15.50- $19.38 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $23.25 per hour.
