Work hours will be Weekend Hours: 9:00 am -9:00 pm on Friday, Saturday and Sunday, for 80 hours per pay period.
To assist the pharmacy, nursing, and medical staff in obtaining accurate home medication lists from patients being admitted to the hospital and conveying that information electronically to the patient record
Accurately Conveying Patient Medication History Electronically Into the Hospital Database.
The incumbent will update the computerized home medication database (EPIC system).
The incumbent must have a thorough understanding of computer functions needed.
Assist the Pharmacy, Nursing and Medical Staff In Obtaining Accurate Home Medication Lists from Patients Admitted To the Hospital.
The basic function will be to obtain accurate home medication histories by patient interview and phone calls to retail pharmacies.
The technician must apply “best practices” when obtaining a medication list including the following:
The home medication list must be reconciled to check for duplications and/or omissions before entered electronically to patient’s record.
All discrepancies must be brought to the attention of the confirming Pharmacist via electronic comments on the EPIC med list or direct communication if available.
The incumbent may attempt to contact the patient’s emergency contact, pharmacy, or physician office to resolve all discrepancies in a timely manner.
Completes Training and Continues To Build Medication and Hospital Process Knowledge
The technician completes competency requirements prior to deadlines, as outlined in hospital policies and/ or legal requirements.
The incumbent will become more familiar with medication reconciliation by reading current Joint Commission requirements and National Patient Safety Goals.
The incumbent will be required to complete a competency checklist, including observing a pharmacist during a patient interview and performing supervised patient interviews before being signed off by the pharmacy manager.
The technician will remain up to date on medications and hospital processes at Finley.
Demonstrates Effective Communication Skills and Collaboration with Patients and Medical Staff
The technician must have strong interpersonal and communication skills to interact with patients and medical staff.
The incumbent must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision.
The incumbent participates in hospital and/or departmental committees relevant to the practice area and medication reconciliation. During medication reconciliation down time, the incumbent is expected to help in the inpatient pharmacy as needed.
Qualified candidates must have a High School Diploma. Certified as a Pharmacy Technician (CPhtT) with registration with the Iowa Board of Pharmacy. This position must keep a current Pharmacy Technician license for the State of Iowa or show evidence of having applied. Previous retail pharmacy expereince is preferred.
