Job Description

Performs basic para-professional nursing or therapeutic work in the care or treatment of residents at a state institution for the elderly, intellectually disabled, or mentally ill; performs related work as required.

Work Schedule: Set rotation of either every other weekend off with days off in between or every third weekend off with days off in between. All RTW's are required to be available to work mandatory overtime with no notice Hours: 2:00pm -10:30pm PM Shift or 10:30pm - 6:15am Night Watch Shift

Available shifts vary depending on vacancies at the time of hire

Description of Work Performed as an RTW: Provide protection from harm and a safe environment for all clients. Intervene and redirect in instances of observable self-injury, aggression, and self-stimulation behaviors using approved GRC techniques. Use good body mechanics to ensure safety. Complete eyes-on accountability and documents. Maintain a safe and clean environment by monitoring the home for needed repairs, assure there are adequate supplies, and maintain proper storage of all chemical supplies, food items and hygiene supplies. Assure that assigned clients have adequate and proper clothing for all occasions and weather conditions.

Provide active treatment by following the clients' daily schedule. Assist with completing Comprehensive Functional Assessments. Understand and implement clients' individual treatment programs/plans and reinforce appropriately. Provide the necessary monitoring, assistance, supervision, and activities to meet the clients' daily living needs. Keep assigned group together, rotate attention and use verbal and/or physical training with supervision during all in and out of home activities. Interact with the clients with sensitivity, dignity, and respect to their emotional wellbeing (tone of voice, privacy, manner you address the clients, listening to them, etc.).

Provide health care services to the clients by providing routine and emergency care as assigned. Provide and monitor personal care to assure client cleanliness and optimal health care, and be aware of clients' health changes and report changes to appropriate personnel. Accurately follow all prescribed therapeutic treatment (physical Nutritional Management Plans, Positioning schedules, Health and Safety Support Plans, treatments, etc.). Reports pertinent health information to the nurse and documents accurately in the electronic medical record. Be knowledgeable of individual health conditions. Obtains vital signs as ordered by physician/nurse and as indicated, accurately records, and reports to the nurse if indicated.

Maintains current certification in first aid. Utilizes hand hygiene practices, personal protective equipment, and follow other infection control/work place practices to provide a healthy and safe environment. Communicate with co-workers, exchange pertinent information, and maintain a positive working relationship with all staff and clients. Participate in staff meetings, discuss clients, policies and procedural changes and involvement in designing programs and in the Individual Support Plans. Follow the supervisory chain of command with job related matters.

Documents entries in the clients' electronic record on assigned people daily (i.e., daily shift response, home visits, special activities, medical problems, maladaptive behaviors, participation in adaptive training, etc.). Record data and information related to the clients' treatment plan (DAR, IPR, data sheets, incident reports, restraints, etc.). Provide quality care as confirmed by Quality Indicators in your treatment team area.

Proactively promote and participate in facility quality assurance efforts designed to improve the quality of life and services of people who live at the facility. This will be accomplished by reading and being knowledgeable of all policies and procedure manuals, the Iowa Department of Human Services Employee Handbook and work expectations assigned by your supervisor. Complete in-service, classroom, online, and recertification courses with at least minimum competency. Implement training and information received from management staff and participate in meetings as directed by management.

Designated positions in this job class require applicants to obtain the required Commercial Drivers License or Chauffeurs License and endorsements within a period of time as determined by the appointing authority at the time of hire.
