JOB DESCRIPTION

Job Summary

Molina Healthcare Services (HCS) works with members, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long-term care, for members with high need potential. HCS staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.

KNOWLEDGE/SKILLS/ABILITIES

  • Functions as a hands-on supervisor, providing direction and guidance to the LTSS service/care coordination team to ensure the delivery of appropriate and quality services in a timely manner compliant with regulatory timelines.

  • Manages staff caseloads and assigns cases appropriately with regard to complexity of medical or psychosocial needs, licensure requirements, and case manager experience (RN, MSW, other allied fields).

  • Audits the performance of team members; provides employee development and recognition; and assists with selection, orientation and mentoring of new staff.

  • Oversees staff use of the electronic case management documentation system in compliance with standard Molina processes, standard documentation styles, and HIPAA. Arranges training as needed.

  • Identifies improvement potential and develops process and procedure improvements with plan leadership

  • Serves as a resource or 'SME' to service coordinators in cases requiring social service interventions, cases with complex needs or the potential for high-risk complications.

  • Monitors clinical and non-clinical team activities to facilitate an integrated care approach with individualized service plans.

  • Participates on internal and external work committees designed to enhance work performance.

  • Works with the Manager to ensure adequate staffing and service levels and maintains customer satisfaction by implementing and monitoring staff productivity and performance indicators.

JOB QUALIFICATIONS

Required Education

Bachelor's Degree in Public Health, Public Policy, Health Administration, Gerontology, Social Work, or related field and/or relevant experience in lieu of clinical experience required.

Required Experience

  • 2 years supervisory experience

  • 2-3 years’ experience in healthcare, long term care, care coordination, case management, and/or discharge planning.

Pay Range: $54,373.27 - $117,808.76 / ANNUAL
