Trinity Health Medical Social Worker II - Skilled Nursing Unit - Full Time - Days in Mason City, Iowa

Position Purpose:

Medical Social Worker II are responsible for addressing psychosocial, financial and discharge planning needs of patients in a variety of settings (i.e. ARU/SNU, home care). This includes, but is not limited to, Advance Directives issues, patient/family advocacy, in-home visits and coordination of community resources. Functions with a high level of independent judgment using skills and knowledge acquired through advanced education and experience to enhance patient care.

What you will Do:

  • Participates in planning related to resident, family, and community interests. Participates in discharge planning, development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.

  • Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing communication with resident and responsible persons of progress/status during stay.

  • Provides information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available. Provide consultation to staff, community agencies, etc., in efforts to solve resident concerns through the development of social service programs

Hours/Schedule:

Full time Days/ 7:30am-4:00pm can be flexible with hours

Minimum Qualifications:

A. Education:

  • Current Iowa LMSW or LISW required

  • Masters in social work degree required

B. Experience

  • Two years professional social work experience in long term care preferred.

  • Community, home health or hospice experience desirable.

C. Special Skills, Competencies, and Aptitudes

  • Knowledge of State regulations and guidelines related to long term care and specifically regarding social worker responsibilities (ie. MDS, discharge planning, Advance Directives)

  • Excellent knowledge and practice of psychosocial assessment skills, including social and emotional factors, counseling for long-range health planning and decision-making, community resource planning and/or short-term therapy, etc.

  • Computer skills, typing proficiency, including Word, Excel
