Position Purpose:

This position accepts the responsibility and accountability for the delivery of patient care during the patient’s stay, through the use of the nursing process. Directs patient/family education and delegates to ancillary personnel while maintaining professional nursing standards.

We are a bridge between hospital and home for patients that are unable to discharge directly home due to therapy needs, extended antibiotics, wound care needs, or IV medications. Our staff care for patients with a multitude of illnesses and needs, some of which include:

Post op Hip Surgery, Post op Back surgery, Post op Knee surgery, Tube feedings, IV antibiotics, Tracheostomy, Anticoagulation therapy, CVA’s, Wound Care, TPN, and t hose needing therapy due to extended illness and weakness.

What You Will Do:

  • Assesses patient’s physical, psychosocial, environmental, spiritual, social and education needs.

  • Develops and implements patient’s plan of care, including the discharge plan based on standards of nursing practice, health center policy, and patient/family needs. Reviews and updates the plan of care based on changes in patient’s condition.

  • Evaluates and documents patient/family responses to nursing interventions while working collaboratively with the interdisciplinary team to achieve desired patient care outcomes.

  • Provides or facilitates learning opportunities to meet the educational needs of patient/family by assessing the learning needs and formulating and initiating a teaching plan to meet those needs.

Minimum Qualifications:

Education:

  • Graduate of a program in professional nursing.

  • Current RN licensure in the state of Iowa.

  • Current RN licensure in the state of Iowa or qualified multi-state license required.

  • Previous experience in acute care preferred.

  • Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

  • Must provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelines
