Case Management Accepts client referrals from and carries out Case Management responsibilities under the direction of the Case Management Coordinator.Accept referral and complete an intake per program guidelines.Complete a comprehensive assessment.Develop a service planImplement services and provide ongoing monitoringConduct at least two (2) HomeMeds assessments per month. Follow-up and reassessment of the client:Monthly client/collateral contact via telephone or in-personAnnual face-to-face reassessment unless needed sooner due to client status changeClient discharge/transferRecords and documentation in the database to include:Intake formComprehensive AssessmentMental Status QuestionnairePersonal Health and Safety PlanReleases of InformationService Plan including signature pageRecord of referrals and requestsCorrespondence related to the caseCase Notes documented in SAMS Journals and Activity and ReferralsEnter Service Deliverables in SAMS for billable and non-billable activityVOCA This position accepts client referrals from community and co-workers, and carries out Elder Rights Specialist responsibilities in the assigned region.Network and coordinate community resources to respond to the needs of elders who are being abused or at risk for abuse or violation of their rights.Develop and maintain community partnerships with DHS, law enforcement, county attorneys, and aging service providers.Take referrals from clients, family members, community partners, and other sources assess potential risk, and respond appropriately.AIRS training requirement. Contact client or request assignment of a case manager to contact the client to assess their needs, identify potential or present risk, evaluate dependency, and coordinate services to reduce or mitigate identified risk factors.Enter all client interactions in the Harmony Database daily and maintain all client documentation.Conduct at least two (2) HomeMeds assessments per month.Maintain confidentiality of all client and agency information, both paper and electronic.Refer stable clients to case management as appropriate; Monitor activity and provide support to the assigned case manager as needed.Collaborate and be a resource for case managers, physicians, law enforcement, county attorneys, DHS, domestic violence agencies, and long-term care facilities.Participate in continuing education, including but not limited to mandatory reporter certification and six hours of aging-related training per year, and agency meetings as required.Options Counseling - Utilize a person-directed approach with the consumer to engage and discuss their goals. Assess needs, values, and preferences to discuss options. Explain the pros and cons of various options and support persons to facilitate decision making.Develop a person-directed action plan to support their decisions.Identify existing consumer resources and supports.Advocate for consumer decisions about services and supports.Identify available resources to address unmet needs.Make referrals or facilitate access to services, including to help complete applications or make phone calls.Enter all client interactions in the Harmony Database daily and maintain all client documentation. Provide follow up to determine how well the action plan is working or if it needs to be revised. Conduct at least two (2) Home Meds assessments per monthMake presentations to consumers, service providers, and other community groups as requested and approved by your supervisor.Attend staff meetings and training events.Performs other duties as assigned. REQUIRED QUALIFICATIONS:Bachelors degree in a human services field (preferred one year of experience to meet AIRS Certification); ORLicense to practice as a registered nurse (preferred one year of experience to meet AIRS Certification); ORLicense to practice as a licensed practical nurse and four years of experience working in the
