About The Job

Assists with MercyOne North Iowa Hospice referrals by serving as a resource to physicians, care management, patients, and families regarding the hospice benefit and coordinating services.

What We Are Looking For

Education:

Current Iowa license to practice as a social worker required

Master’s degree in Social Work

Hospice certification desired

Experience:

Five or more years of experience in a health care setting.

At least two years experience in Hospice Care

Experience and ability to exercise independent judgment, yet lead and/or work within a team relationship

Special Skills and Competencies

Demonstrates knowledge of and belief in the hospice concept

Demonstrates knowledge/understanding of hospice criteria, appropriate symptom control measures and reimbursement mechanisms

Demonstrates knowledge of hospice practices and trends

Demonstrates knowledge of diverse lifestyles, family structures and dynamics; cultural and religious practices; learning preferences, and age/gender specific considerations

Utilizes computers (HCHB, Halo & Ensocare) and information systems

Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.

Will need to operate Mercy Vehicle X Personal Vehicle N/A

Frequency of operation: Xdaily _ weekly monthly __quarterly
