About The Job

Provides skilled nursing services to patients and families at end of life including assessment, planning, intervention and evaluation in their home setting. This position works during the day and assists with call schedule for nights, weekends, and holidays. The Hospice Home Care Nurse is part of the interdisciplinary team that helps patients and their families remain in their home setting during their journey.

What We Are Looking For

Education [formal]:

  • Current RN license is required

  • BSN preferred

  • Hospice and Palliative Care Certification preferred


  • Must have demonstrated nursing competence in a health care setting for two years

  • Caring for terminally ill patients/families, Hospice or home care nursing experience is preferred

  • Special Skills and Competencies: Critical thinking skills, communication skills, ability to problem-solve and work within a team environment.

  • Must demonstrate the ability to multitask in a fast paced environment with flexibility and strong organizational skills

  • Computer skills/typing skills, and the ability to use electronic methods of communication

  • Obtain Port Access Certification within one year of hire date

Completes the following mandatory courses :

  • Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

  • Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.

Motor Vehicle:

  • If required to operate Mercy vehicle, will need to have proof of and maintain valid driver's license on file with Human Resources.

  • Note any special licenses or requirements for Mercy vehicle operation (i.e. CDL) ______

  • If required to operate personal vehicle, will need to have proof of and maintain valid driver's license and adequate insurance coverage on file with Human Resources
