Trinity Health RN Nurse Educator - Diabetes & Endocrinology in Waterloo, Iowa

Description:

Provides assessment, teaching and follow-up instruction to program participants in individual and group settings.

• Receives referrals and order from physician.

• Conducts initial needs assessment.

• Provides education appropriate to individual (i.e., age, reading level, language, disability).

• Coordinates/provides follow-up services in person or via phone.

• Receives and returns calls from patients in a timely manner.

Communicates education provided and patient progress.

• Communicates with physician regarding education provided, follow-up plan.

• Documents clearly and concisely and in accordance with policy.

• Identifies changes in patient status and communicates to physician in a timely manner.

Facilitates ongoing development of diabetes services.

• Reviews and updates educational materials/curriculum.

• Collects and reviews patient satisfaction data.

• Assists with the formal review of program on an annual basis.

• Serves as a member of the Diabetes Advisory Committee.

Provides a variety of clerical support services.

• Maintains adequate inventory and supplies.

• Maintains filing system.

• Assists with annual data collection for certification.

• Compiles necessary documentation for reimbursement as needed.

• Types memos/correspondence as needed.

Case Manager for Diabetes Education

• Answers phone calls from patients having problems with diabetes during scheduled visit.

• Coordinates diabetes care with PCP to assist patients on the phone with diabetes problems.

• Documents phone calls in electronic health record. monitor care coordinator
