Job Description:

The Population Health Manager is responsible for ambulatory care coordination and patient-centered care strategies within the health system. Evaluates and promotes best practice in value-based care quality, cost and utilization. Coordinates interdisciplinary approaches to chronic care management, outreach activities, patient engagement, remote patient monitoring and community needs assessment. Focused on improving the health of our community through high- quality care, data analysis and evaluation of programming needs, and monitoring health disparities and health outcomes.

Qualifications:

Education and Experience

  • Prefer graduate of an accredited school of nursing or bachelor’s degree in healthcare related field

  • Prefer minimum of 2-3 years hospital/clinic experience
