General Job Summary:

Demonstrates and promotes servant/participative leadership and provides oversight of the Social Services department by building relationships, acting with integrity and humility, modeling the way, listening with the intent to be influenced and embracing change.

Duties and Responsibilities:

The Director of Resident and Family Services is responsible for the following duties and responsibilities which are considered essential functions of the position.

v Discharge Planning: complete the initial social assessment and reassessment of each resident participate in interdisciplinary care planning process and discharge planning, assist resident and family in applying for assistance from agencies such as Home Health Care, etc. when a resident is discharged, if such assistance is needed.

v Counseling: conduct orientation program for new residents, provide counseling to residents and family members in facility adjustment, dealing with losses, death and dying, the aging process, and provide information and assistance to resident and family member in application for veteran’s benefits, Medicaid, and other services to which they may be entitled.

v Advocacy: intervene on behalf of resident to help resolve conflict or complaints regarding roommate, staff members, or any other concerns, and will inform residents of their rights under Resident Bill of Rights.

v Education: provide in-service education to facility staff and community groups.

v Investigate self-reports and determine the appropriate course of action.

v Collaborate with nursing on resident care planning.

v Complete MDS and VA required documentation.

v Knowledgeable of the functions and operations of the facility and their capabilities to provide clients with the appropriate health care.

v Develops positive working relationships with external partners of the facility by informing and educating the community on the facility’s availability and services.

v Provides to the Administrator a list of contacts on a weekly basis with feedback on services the facility provides or could provide.

v Participates in marketing management by collecting and organizing data on all client referrals, admissions, denials, and reasons denied.

v Identifies positive and/or negative trends and reports to Administrator.

v Develop special marketing projects utilizing evaluations and surveys.

v Follows proper procedures for hand washing, isolation, PPE, universal precautions, disaster drills, hazardous conditions, and safe operations of equipment.

v Ensures a safe environment.

v Promptly reports any unsafe working condition.

v Reports any accidents to manager or supervisor on duty ASAP.

v Demonstrate knowledge of fire, general safety, and disaster plan procedures.

v Demonstrates knowledge of proper body mechanics.

v Operate all equipment in a safe manner.

v Utilizes cost effective measures during job performance.

v Follows set budgetary guidelines.

v Interact/communicate with residents, staff, and visitors in a courteous manner.

v Acts as a positive representative of the facility.

v Communicates with employees in all departments.

v Keeps all resident and employee information confidential.

v Reports all complaints/grievances to the Manager or Supervisor on duty.

v Reports incidents or suspected incidents or resident abuse to the DON/Designee immediately.

v Ensure that all residents are treated fairly, with kindness, dignity, and respect.

v Knowledgeable of all resident’s rights according to facility policy and regulations, including the right for a resident to refuse treatment.

v Informs nursing and rehabilitation staff of any incidents/problems.

v Maintains positive, professional interaction with residents and staff.

v Participates in Quality Assurance Programs and any other facility committee, or program which seeks to improve the performance and accuracy of resident care.

v Participates in facility focus group meetings as desired or requested.

v Understands and complies with the Corporate Compliance Plan.

v Other job duties assigned as deemed necessary by the Director of Nursing and/or Administrator.

v Complete PASRR required documentation.

v Contribute toward improving the Quality Improvement process and survey readiness to achieve successful outcomes.

v Abide by established policies of the facility and interpret them to concerned parties.

v Remain current in the field of Social Services through publications, workshops, and institutes.

v Exercise care in the use of supplies and equipment.

v Maintain resident information about Pine Acres internal affairs in strict confidence.

v Participate in In-service education.

v Follow Pine Acres policies concerning personal grooming, appearance and appropriate dress code requirements.

v Preserve resident dignity by always knocking on resident room doors before entering.

v Ensure Activity programming best meets the needs of the residents.

Qualifications:

v Education

o Bachelor's Degree is required. Must be able to read, write, speak, and follow oral and written communication in English.

v Work Experience

o Minimum of one year experience in a health care setting working directly with people. Thorough knowledge of the case work process, medical and public health settings, principals of public welfare, and a working knowledge of social and health resources of community.

v Lifting Requirements

o The Resident & Family Services Director is frequently called upon to lift items up to 10 pounds. The Resident & Family Services Director will occasionally be called upon to lift objects between 10 and 25 pounds. Lifting in excess of 25 pounds is not required.

v Carrying Requirements

o The Resident & Family Services Director is frequently called upon to carry items up to 10 pounds. The Resident & Family Services Director will occasionally be called upon to carry objects between 10 and 25 pounds. Carrying in excess of 25 pounds is not required.

v Sight Requirements

o The Resident & Family Services Director is required to have 20/20 correct vision.

v Hearing Requirements

o The Resident & Family Services Director is required to have normal range of hearing, as corrected

v Socialization Requirements

o The Resident & Family Services Director is required to maintain good socialization skills in and around resident areas and with other co-workers; maintain a friendly, caring and cooperative attitude; be able to respond calmly under stressful situations. The Resident & Family Services Director must have imagination and creative ability and a sympathetic approach to illness.

v Other Physical Requirements

o The Resident & Family Services Director will regularly be required to stand, sit, and walk. The Resident & Family Services Director will occasionally be called upon for squatting, kneeling, bending, and crouching.
