Immanuel Participant Center Supervisor - Full Time Days - Immanuel Pathways Central Iowa in Windsor Heights, Iowa

Manages the daily operations of the Immanuel Pathways Adult Day Health Center. Responsibilities include the planning, coordination and implementation of all activities in the center, management of staff, and compliance with internal and external standards and regulations. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

Management of Center 50%

  • Maintains the stability and reputation of Immanuel Pathways by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing Immanuel Pathways operations.

  • Directs, supervises, performs initial and annual competencies on, and evaluates the performance of the Participant Center Aides as appropriate.

  • Provides/coordinates in-service and on-the-job training for Participant Center Aides according to program needs, and state and federal regulatory requirements.

  • Ensures that the job responsibilities, authorities and accountabilities of all direct reports are defined and understood.

  • Fulfills all administrative requirements, such as development of Participant Center Aides’ schedule, oversight and completion of daily-required documentation and reports associated with State and Federal agencies.

  • Recommends changes in service delivery and staffing as required by the development and growth of the agency.

Compliance 45%

  • Responsible for maintaining compliance with all State and Federal program requirements, obtaining and maintain certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.

  • Functions as a member of the Interdisciplinary Team (IDT). Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for 24-hour care delivery.

  • Maintains the program space and its equipment as required by plant operations and state and federal agencies.

  • Monitors operations to ensure consistent compliance with established quality assurance standards.

  • Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.

  • Acts within the scope of his or her authority to practice

  • Monitors Immanuel Pathways’ operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.

  • Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.

  • Professional Activities

  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

  • Establishes positive working relationships with health and social service providers.

Other 5%

  • Performs related duties as required or requested.

  • Acts as facilitator, when designated, of IDT Plan of Care meetings.

Education-

  • Associate Degree in the Health Care field required.

  • Bachelors Degree in Health Care preferred

Experience-

  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.

  • Two (2) years of management experience in a “center” or program-centric organization, such as YMCA, adult participant center (preferably in a geriatric care setting) is required.

  • Equivalent years of education may substitute for experience requirement

  • CNA state required training

Other Requirements-

  • Agree to abide by the philosophy, practices, and protocols of the PACE organization

  • Job specific competencies for the Participant Center Supervisor will be met prior to assuming participant care.

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

  • Reliable transportation is required as this position may require travel between locations.

  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).

KSA- Knowledge Skills and Abilities-

  • Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.)

  • Knowledge of the PACE regulations and Immanuel Adult Day Health Licensing regulations

  • Knowledge of healthcare and aging networks

  • Knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families

  • Knowledge of quality improvement and cost containment systems

  • Knowledge of local health care and geriatric service networks

  • Skilled in decision-making

  • Skilled in supervising employees

  • Skilled in written and oral communication

  • Ability to foster collaborative working relationships

  • Ability to apply creative problem-solving skills

  • Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities.

  • Ability to chart via Electronic Health Records

  • Proven experience and basic computer proficiency (internet, email, Microsoft Office)
